Effective 7/1/10 Moffat County School District is Self-Funding its Health and Dental Insurance. We have contracted with NGS Core Source as our Third Party Administrator(TPA), Cofinity and Private HealthCare Systems Multiplan( PHCS) as our network providers, and Partners RX as our Phamacy Benefit Manager. Vision Insurance
Vision insurance is voluntary and is not included as part of your health and dental insurance plan. You must sign up separately from the health and dental plan to have vision coverage.
If you choose to have vision insurance, VSP (Vision Service Plan) is our provider.
